The Office of the Commissioner, Public Health and Medical Education, Madhya Pradesh has issued an important notice for candidates participating in the Madhya Pradesh State Level Combined NEET MDS Counselling 2026.
The notice, dated September 7, 2026, concerns the bond affidavit to be submitted at the time of admission to postgraduate dental courses (MDS).
According to the notice, candidates who wish to exercise the upgradation option in subsequent counselling rounds must submit an undertaking in the prescribed manner regarding the rural service bond and seat leaving bond.
Candidates admitted through the First Round who wish to take the upgradation option in the Second Round must execute an affidavit in favour of the Principal of their concerned dental college.
Similarly, candidates admitted through either the First Round or Second Round who wish to opt for upgradation in the Mop-Up Round will also be required to submit the prescribed affidavit.
The affidavit must be executed on INR 500 stamp paper.
Through the affidavit, candidates will undertake that if they are not upgraded, they will execute the required bonds at their already admitted college.

The required undertaking must be executed through a properly stamped and notarized affidavit, as prescribed by the Directorate of Medical Education, Madhya Pradesh.
The notice specifies that the relevant bond affidavit is to be stamped according to the prescribed bond amount, at 0.5% of the bond amount.
Candidates seeking upgradation should therefore ensure that the required affidavit and bond-related formalities are completed according to the instructions of the concerned institution.

Candidates should contact their concerned college for completion of the required bond and affidavit formalities.
Candidates who intend to participate in further upgradation rounds should complete the required affidavit and bond formalities before exercising the applicable upgradation option.
Candidates should also carefully read the instructions issued by their allotted college and the Directorate of Medical Education, Madhya Pradesh, regarding the execution of the rural service and seat leaving bonds.
