The Office of the Commissioner, Public Health and Medical Education, Madhya Pradesh, has issued an important notice regarding the bond documents required during admission to postgraduate medical courses under Madhya Pradesh State-Level Combined NEET PG Counselling 2026.

According to the notice dated November 2, 2026, candidates who have already taken admission and wish to participate in the next upgradation round must submit an affidavit on INR 500 stamp paper. The affidavit must be executed in the name of the Principal of the concerned medical college.

Which Candidates Must Submit the Affidavit?

The requirement applies to the following candidates:

• Round 1 admitted candidates: Candidates admitted through Round 1 who wish to opt for upgradation in Round 2.

• Round 1 and Round 2 admitted candidates: Candidates admitted through either of these rounds who wish to opt for upgradation in the Mop-Up Round.

These candidates must complete the prescribed affidavit formalities to exercise the upgradation option.

What Must Be Mentioned in the Affidavit?

Candidates must submit a notarised affidavit on INR 500 stamp paper, addressed to the Principal of their respective medical college.

The affidavit must state that if the candidate does not receive an upgraded seat, they will execute the required Rural Service Bond and Seat Leaving Bond separately at the college where they have already taken admission.

The bonds must be duly executed on notarised affidavits stamped at 0.5% of the respective bond amount, as specified in the notice.
